In the event of a data security incident, such as an inadvertent disclosure or system compromise, the Project Director, listed contact(s), organizational representative, or CISO must immediately contact the SPARCS program detailing the nature of the incident and the data involved.
Please provide the following information:
- If a cybersecurity security incident did occur?
OR - An incident has been confirmed, and the investigation is underway to determine if SPARCS data was affected.
- Confirm the number of SPARCS data years that were affected.
- Other relevant information to the incident.
- Contact Information for the organizational representative handling the incident response.
