If you need to add or remove data users, submit a request through our SPARCS Support ticket system
Please include the following:
- SPARCS application request #
- For new user(s) include (First/Last Name; Organization; Research Role) & completed DUA (DOH-5131-IA form)
- For removing user(s) include (First/Last Name; Organization; Research Role). Adding/removing users does not require a DOH-5132 submission.
Users are only approved upon confirmation from the SPARCS program. You must await SPARCS decision before providing the user with internal access to your approved SPARCS data.
Note: Individuals who will have access to the raw dataset(s) or data output(s), that do not follow the small cell size policy, should be listed and sign individual DUAs. This can include external research partners, contractors, and information technology (IT) staff. IT staff that maintain the technology environment, and are not direct data users, do not need to sign Individual DUAs.
