To submit a request for an exception or extension from SPARCS compliance, use this template and send to [e-mail]. Exception and extension requests are reviewed and processed monthly. Depending on when you submit your request, your approved extension or exception may not appear on that month’s Compliance Reports that are posted on the NYS DOH website.
